I suspect not mate BUT not had any experience TBH so maybe someone else will have a different suggested diagnosis and advice… ?
I am interested to see what else is suggested.

i would say a tumour also,
plenty of video's on youtube showing removal of far more difficult ones on koi,
but would imagine this would need to be done by a vet though, as it may need cauterising...?

Slightly unrelated but I used to fish a lake Layer pits in Colchester, and there was a period where a few of the fish had growths like these out of their heads. They are have gone now but was a few months or more where I had a few like it. Don't know if they ended up dying or just dropped off, common/mirror carp are a lot more hardy than koi after all. There was never any reasoning behind it though.

Hi,
If you contact me, I can remove the growth. It looks like infected carp pox, but I would need a closer examination to be sure. I am in Darlington.
Contact details can be found at The Koi Clinic - Koi Fish & Pond Health Consultancy UK
Running heater at low temp?
Covering the pond with polycarb should be enough to stop water temps dipping below 6C for the most...
