Hi Craig,
Great pic of an unfortunate situation.
Only time I have seen similar was on a very knowledgeable friend of mine’s koi who believed something had got in and caused the inflammation. What he did was extremely specialist which I would not advise attempting without prior consultation with say Paula Reynolds (if she is available) or Manky Sanke on here as he may well have seen similar previously.
Cheers

so the bigger ones fat cheek seems to be going down. but the little guy (pictured above) seems to be going more red now. hes also sort of isolating himself (though is still coming over for food when i am feeding, and is still eating)
I am still adding bicarb to try and get up/over the level indicated by the test kit. I am up to 6 drops now on the test before going yellow, GH however does not appear to be improving.

Could you sedate it Craig and have a look inside the mouth to see if anything is stuck / damaged on the inside
of the cheek....Do you feed dried mealworms ? sometimes they can have really sharp ends on them i have a quick look
now just to be sure there's no sharp ends on them
Running heater at low temp?
Covering the pond with polycarb should be enough to stop water temps dipping below 6C for the most...
