Could be their gills are burned from the ammonia. Even after your water is perfect you could still experience longer term losses. Ammonia burns and does damage that takes weeks to heal.

That much ammonia and nitrite is enough to kill fish. It stresses them making them susceptible to other issues. You should have 0 nitrites.! Use ammonia lock, big water changes and feed every other...

You’re wrong! Sorry but that’s overstocked for Koi. Sounds like a swim bladder problem, probably some infection. Have you taken levels? Ammonia. Nitrite etc ?

Way to small of a living space and sorry to say, it may be just the dirty glass but the water quality looks poor. Koi don’t belong in a tank. Stick to goldfish.
