Horrible when your fish get stuff nobody has seen before.... someone must have had something similar surely?
I think that is in the region where the intestines/gut and ovaries are, I'd say more likely the former.
At a guess maybe worth seeing if the koi is pooing properly and check the colour which could help check for signs of constipation.
This document says there's a urinary bladder there.... could that be blocked? Would salt help if that's the case?

All just guesses really, trying to think of something that might help.

Been in a vat since a found it turned filter off for a little bit still with air just to try and see some waste long clear and floating . . But the new thing is now I know I've turned up for work a bit green around the gills after to much scotch but this is taking it to extremes

The only thing I've seen like that before was a tumour .... but I've got no idea what that is in your poor fish.
I feel your pain though was just going through that unknown fish health problem myself yesterday with a weird "rotting" doitsu scale condition.
I don't know for sure what I would do myself in your case ... possibly a salted and heated QT as I've found that helps in just about any dilemma (unless it requires a treatment not compatible with salt) ... fingers crossed for a diagnosis ...

Ok so had a look in the dry definitely like a popped blister even down to some clear and some blood red gunk coming out just as a blister would . Moving more now but if that's the temps increase or this blister thing ????

Sitting on the bottom . Swimming looks a little easier now was very laboured before not eating but starting to fight when you pick it up where as before it just lay there and dont try to wriggle free

Have you had any response from the health pro's yet?
Where have you got the fish just now? what temp is it at? and have you added any salt? ... I think you could salt to 0.3% no problem and the fish would be happier ...
